what is the address and phone number of the New Jersey Banking Commission?

Has anyone ever had a bank’s bond headquarters misplace their application for a savings bond and if so, what was done to help find it or return your money?
A bank has lost mine and I am constantly calling without any success. The bank keeps telling me they’re researching it.
